Pages from my website are indexed by my server's IP too in Google earn by blogging adsense income

Hii r/SEO members,

Recently, I discovered that 2 versions of my web pages are indexed in Google. First - www.xyz.com/seo-issue/ and second 123.123.01.02 /seo-issue both pages have canonical tags claiming to be the original source of the content. I understand that is a major issue but I don't know how can it happen? What do you all suggest to fix this issue?
