How do you ask someone to be your mentor? entrepreneur how earn by blogging blog

I'm a woman in my early thirties and I'm looking to move into a formal leadership role. I work in software development where it's cool to have flat organizations (which I love, by the way) so leadership is often informal and I do plenty of that. There is an executive at my company who I really admire and I want to emulate. What is the best way to ask her for mentorship?
